Overdiagonal subgroups of the hyperbolic unitary group for a good form ring over a field

In the hyperbolic unitary group over a field the lattice of subgroups containing all diagonal unitary matrices, has the standard description. The corresponding form ring has to satisfy some conditions, in particular for fields of characteristic 2 we don't consider the symplectic group and the defective orthogonal groups.
